Cigarette/Tobacco/Electronic Vaping Device License
Cigarette/Tobacco/Electronic Vaping Device License Applications are available on the Department of Revenue website and must be submitted to the City Clerk’s Office. The following three forms must be completed for submission:
- Cigarette, Tobacco, and Electronic Vaping Device Retail License Application (CTV-100)
- Cigarette, Tobacco, and Electronic Vaping Device Retail License Individual Questionnaire (CTV-101)
- Cigarette, Tobacco, and Electronic Vaping Device Appointment of Agent (CTV-102)
Cigarette/Tobacco/Electronic Vaping Device Licenses expire June 30th of each year.
